Privacy notice

The general contact, referral, and get-involved forms collect only the contact and non-clinical details shown on each form. The separate signed-in adult intake can collect identity, contact, care-needs, medication, allergy, coverage, safety, and acknowledgment information after account verification.

General submissions are stored so authorized team members can review and follow up. Protected intake records are used only for intake review, tracked follow-up, assignment, and a documented transition to next-step contact. Account information supports authentication and access control.

Public visitors cannot read submissions. Patients can access their own intake, administrators can review submitted records, and team members can access only records assigned to them. Intake submissions are locked after submission, with later responses recorded as amendments. This staging website does not accept payments or donations.

Formal retention, correction, deletion, and legal-hold rules have not yet been approved. Real-patient use remains disabled until those rules and the organization’s privacy procedures are in place.

Use the public forms only for non-clinical requests. To ask a privacy question, use the general message form on the Contact page without adding health information.
